Summary of Essential Functions for the Position:
Position is responsible for being a liaison to discharged patients to ensure they are satisfied with care provided and all follow up has been provided from agency. Position is to advocate as liaison for any outstanding needs, as identified, to the primary care physician. Communication may occur with multiple internal and external departments from time to time.
Qualifications / Licensure / Certification / Knowledge / Skills / Abilities:
Previous experience in healthcare/homecare, preferred. Requires the ability to exchange routine information in an appropriate manner requiring good oral and written communication skills. Position requires good organization skills, time management, customer relation skills, ability to multitask, and competent with computer applications required. Prior experience with Microsoft Excel or Google preferred.
High School Diploma or GED. Licensed Practical Nurse preferred.
Working Conditions & Physical Requirements:
Works indoors in agency office. Most of the day is spent sitting. Requires minimal physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and minimal assist in lifting 20 pounds. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
